Amritha Aiyer began her career as a lead actress in the movie Padaiveeran (2018) where she starred alongside Vijay Yesudas, playing the role of Malar. Her performance was well-received, with a review from The Hindu noting that she fit her role aptly. Following this, she appeared opposite Vijay Antony in the movie Kaali (2018).

In 2019, Amritha was set to make her debut in Kannada cinema with the film Gramayana with Vinay Rajkumar. However, the project was halted due to the producer contracting COVID. Nonetheless, Amritha played a notable role as Thendral, the captain of the Tamil Nadu football team, in Atlee's Bigil (2019), sharing the screen with Vijay.

Aiyer ventured into Telugu cinema in 2021 with the film Red, where she acted opposite Ram Pothineni under the direction of Kishore Tirumala. Later that year, she had another release�Vanakkam Da Mappilei�opposite G. V. Prakash Kumar, available directly on Sun NXT. Her third release of the year was Lift, which initially opted for a theatrical release but was eventually released on Disney+ Hotstar instead.

In 2022, Amritha had a single release, the Tamil film Coffee with Kadhal, opposite Jai. Subsequently, she was involved in the Telugu film Hanu-Man alongside Teja Sajja that was a blockbuster and an untitled Tamil-Telugu bilingual film with Teejay Arunachalam. Hanu-Man premiered on January 12, 2024.
