Karthika Nair made her film debut with Josh in 2009. The film was directed by Vasu Varma and produced by Dil Raju. It was a Telugu political action drama. The movie introduced both Naga Chaitanya and Karthika in lead roles. J. D. Chakravarthy played the antagonist in the story. Music was composed by Sandeep Chowta. Looking back, Karthika once said, “It was a special beginning because I got to start my journey in cinema with a strong story.”

She was last seen in Purampokku Engira Podhuvudamai in 2015. The film was directed by S. P. Jananathan. It featured Arya, Shaam, Vijay Sethupathi, and Karthika in key roles. The story focused on a communist sentenced to death and the struggle to save him. Reflecting on the film, Karthika shared, “It gave me a chance to work in a project with depth and meaning.” This movie marked her last role in cinema.
